aspiring futures SA is seeking a Experienced Service Coordinator
The Service Coordinator will oversee the planning, coordination, implementation and administration of programs and staff that support participant needs within the framework of NDIS. The Service Coordinator is responsible for developing community and participant relationships that contribute to person-centred outcomes within the strategic directions of aspiring futures SA.
The Service Coordinator is also responsible for facilitating the implementation of the behaviour support plan and developing effective and creative ways for the support team to access information.
Key Responsibilities of the Position:
- Liaising with external stakeholders who are involved in participant care – which may include Allied Health professionals, GPs, and other community organisations.Coordinating support for participants with these services as needed.
- Ensure all stakeholders are clear on the agreed support plan and maintain positive working relationships with internal and external stakeholders.
- Ensuring that all service agreement conditions are adhered to, and all services continually meet the NDIS Commission safeguarding standards and quality frameworks.
- Participating in program development and delivery, adhering to the PBSP and RP implementation, promoting data collection and person-centred approaches
- Monitor and plan for changes in supports, capacity, staffing and participant support needs. Monitor program budgets and funding, ensuring that is occurring and liaising with finance to ensure that information is shared appropriately.
- Provide on-call support to programs operating out of business hours as part of a roster.
- Managing day to day running of participant's houses, rosters, documentation and recording of accurate information.
- Actively managing incidents and offering behaviour support strategies and expertise to diffuse situations that may arise.
- Identify and monitor risks and undertake participant assessments
- Carry out Interviews with potential new support staff, and assist with the onboarding process
- Engage Service Coordinators and support workers in performance reviews and upskill leadership capability through mentoring.
- Ensuring staff are supported in the delivery of person-centred, best practice services to all registered participants.
Your application will be assessed against the following Mandatory Requirements:
- Tertiary qualifications in social worker, psychology, mental health, allied health or disability services or other relevant qualifications; strong preference will be given to applicants with clinical/nursing/allied health qualifications and experience.
- Experience in coordination and delivery of quality disability care in a community setting and with external providers.
- Experience with rostering and coordinating supports.
- Experience with managing complex behaviour participants
- Empathy, understanding, ability to communicate and experience of working with people living with multiple and complex issues.
- High standard of computer skills, knowledge of MS Office and ability to learn new computer software programs and database.
- High standard of verbal and written communication skills.
You will have a positive work commitment and a can-do attitude, demonstrated ability to problem solve a variety of situations and proven organisation skills and ability to prioritise multiple tasks to meet required deadlines.
