Clinical Care Manager (8 Month Parental Leave Cover)

A rare opportunity to lead the clinical care team at Aeralife Northcote for an 8-month Parental Leave cover.

As Clinical Care Manager, you will be a key member of the leadership team, with responsibility for all clinical matters and you will partner with the Residence Manager to ensure exemplary resident care is delivered and maintained.

You will enjoy and be fully supported in the area of Quality, Governance and Risk, Operations including IT, HR, Asset Procurement, Finance and Marketing.

The Opportunity on Offer

The Clinical Care Manager (CCM) is responsible for delivering personalized care that meets the individual needs and preferences of residents. This role involves working closely with residents and their representatives or medical decision-makers to oversee clinical care and manage complex health needs. They supervise staff to ensure high-quality care and compliance, while leading clinical teams and coordinating communication among health professionals.

Requirements:

  • Responsible for the provision of care tailored to the residents' individual needs and preferences
  • Engaging with residents, representatives / medical decision maker to deliver and supervise clinical care and complex health needs for all residents.
  • Drive clinical outcomes and performance from the Clinical and Care teams, supervising staff to maintain a high quality of care and compliance through assessing, planning, documenting, implementing, and evaluating support and interventions.
  • Be a role model of clinical leadership through accountability and provide a reliable and informed flow of information ensuring all health practitioners, clinicians and allied health services are engaged appropriately.

What you Bring to the Role:

  • Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) experience in clinical care systems and reporting
  • Minimum three (3) years' experience in aged care
  • Analytical approach to problem solving and reporting.
  • Strong time management skills
  • Sound understanding of the Aged Care Quality and Safety Standards
  • Excellent computer literacy and competency in electronic care systems
  • Accomplished English written / verbal communication skills.
  • Current NDIS Worker Screening Check
  • Current Covid-19 Vaccination highly recommended
