- The Support Coordinator manages a portfolio of participants including all aspects of their service experience and is the key contact point for customer relationship working collaboratively with internal stakeholders to identify customer preferences and match these with appropriate staff & supports needed for participants.- This position is responsible for the implementation of participant plans that meet the needs and expectations of participants whilst coordinating the provision of high quality, individualised and responsive support services this requires the coordination and supervision of Community Support and/or Care Workers and working collaboratively with the individual, their informal and formal supports, and other stakeholders.This is a permanent full-time position available to work in a hybrid working model based at our Camberwell offices in Melbourne,**About You**
