BASScare is a NFP organisation that takes great pride in our aged care, community and disability services in Melbourne’s Boroondara and surrounding LGAs. Are you an experienced Dementia Services leader ready to work with highly experienced Dementia Services Team?

**The Role**:
Reporting to the Community Services Manager, you’ll lead the Dementia Services team working across all three centres, ensuring support for the staff. This role is responsible for service delivery, staff management, program planning, volunteer coordination, client care, and addressing operational issues.

**What You’ll Do**
- Oversee daily program delivery at each centre.
- Ensure programs are delivered in line with client needs, care plans and legislative requirements.
- Liaise with Program Operations Lead to ensure smooth scheduleing and resource availability.
- Provide leadership and support to Program Assistants and Volunteers
- Provide on the job coaching to Program Assistants
- Provide regular performance feedback, and conduct team meetings
- Facilitate training and mentoring to support staff development
- Coordinate activity planning with a focus on quality, relevance, and client engagement
- Adapt programming based on client and staff feedback, attendance, and outcomes
- Ensure risk assessments and safety plans are in place for all activities
- Support the Program Delivery Lead in the completion of annual reviews for all clients
- Engage with clients and carers to understand goals, preferences, and needs
- Oversee day-to-day housekeeping in collaboration with staff
- Manage incidents and complex client concerns, including escalation when necessary
- Build and maintain strong relationships with carers and family members
- Provide information, support, and referrals as appropriate
- Ensure carers are included in care planning and decision-making processes.

**About You**

You are an experienced Dementia Services Leader with relevant experience preferably in the Aged Care or Community Sector.

Essential
- Relevant qualifications in Aged Care, Community Services, or a related field
- Proven experience coordinating community programs and leading teams
- Skills: Strong leadership, interpersonal, conflict resolution, and communication skills
- In-depth knowledge of dementia care and person-centred practice
- A current Police Check
- A full driver’s license
