HP5 Team Leader - $130,149 - $135,803 per annnum
NG7 Nurse Unit Manager - $128,701-139,732 per annum
The Nurse Unit Manager Team Leader provides expert clinical leadership and management capability within the Preventative Integrated Care Service via effective operational and resource management. The Nurse Unit Manager Team Leader positively influences the outcome of patient centred care through effective management skill, support and role modelling contemporary best practice and evidence, team building and guiding clinical care and service delivery.
Preventative Integrated Care Service (PICS) will provide a rapid response (review within 24-48 hours) for intensive specialised medical management, delivered by a team of medical, nurse practitioner and allied health members with a disease specific focus through flexible and varied modalities. This service addresses a gap for a target population of high-risk chronic condition patients requiring specialist intervention and coordinated care in their most vulnerable periods to improve quality of life, avoid unplanned admissions, prevent possibly preventable hospitalisations (PPH) and reduce length of stay
About you
- Demonstrated contemporary clinical nursing expertise with clear understanding of the requirement for patients with chronic conditions.
- Demonstrated ability to build and maintain strong relationships, strong teams within a framework of open communication and the establishment of a positive environment across the broad clinical interprofessional team, actively contributing to the learning and development of others.
- Demonstrated management experience including financial literacy and capability with an ability and commitment to deliver clinical services collaboratively integrating the broader multidisciplinary health care team in delivering high quality, cost effective, patient centred care.
- Demonstrated skill in financial management, data analysis, outcome evaluation, service improvement and the ability to communicate these effectively to senior management or external agencies.

**Mandatory Requirements**:

- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
- Vaccinated against COVID-19 as outlined in the Health Employment Directive NO.12/21and the Queensland Health Human Resources Policy B70.
