**The Role**

As a crucial member of our team, you will play a dual role in both leadership and case management, combining clinical governance oversight with hands-on support.

You will enhance performance across all business sites including high physical support and psycho-social mental health, ensuring compliance with regulatory standards and delivering consistent, quality care to participants with a range of complex health needs.

This role brings positive change to countless lives within our community.

**Responsibilities**
- You will ensure high-quality care and drive NDIS-compliant practices and comprehensive health assessments, updating care plans to meet individual physical and mental health needs.
- Support clinical governance and oversee Aged Care and CHSP programs.
- Design and deliver clinical training for support workers and leaders.
- Cultivate trust with clients, families, and stakeholders, offering clinical guidance to achieve positive health outcomes.
- Champion Client-Centered Care. Regularly review care plans, ensuring client preferences, independence, and regulatory compliance are at the forefront of care delivery.

**Skills & Experience**
- Bachelor of Nursing (or equivalent) with at least 2 years' experience in disability, aged care, or allied health services, including hands-on training.
- Experience in competency-based training and assessment, with the ability to plan, implement, and evaluate service plans.
- Ability to work in a fast-paced, dynamic environment and interpret clinical reports, policies, and processes.
- Valid NSW Driver's License, with access to a car and car insurance.
- Strong understanding of disability, dual diagnosis, and complex client needs.
