We are seeking a dedicated, compassionate, and open-minded Support Coordinator to join our team in the South East region. In this role, you will play a pivotal part in empowering individuals with disabilities by providing personalised, high-quality support throughout their NDIS journey.

**What We’re Looking For**
Strong understanding of the NDIS
Previous Support Coordination experience (preferred)
Knowledge of SDA and SIL (desirable)
Relevant qualifications (e.g., Cert IV in Disability, Community Services, Mental Health)
Experience with psychosocial and intellectual disabilities
Excellent written and verbal communication
Strong computer and report-writing skills
Ability to work independently and manage time effectively
Willingness and ability to travel for client visits and monthly meetings
A reliable vehicle and current Australian Driver’s Licence
Valid WWCC and NDIS Worker Screening (or in progress)

**What You’ll Be Doing**

As a Support Coordinator, you will assist NDIS participants to navigate the NDIS system, engage with the right supports, and build capacity for greater autonomy. Your role will include:

- Assisting participants in understanding and implementing their NDIS plans
- Coordinating formal and informal supports
- Supporting participants with psychosocial and mental health needs
- Managing crises and liaising with key services and stakeholders
- Supporting access to health, housing, and disability services
- Writing professional progress and support reports
- Working remotely and managing a flexible schedule
- Travelling within your allocated region (South & South East)

**Job Type**: Casual

Pay: $28.50 - $57.55 per hour

Expected hours: 24 per week

Schedule:

- 8 hour shift
