**About the Role**:As a Service Delivery Coordinator, you will play a crucial role in ensuring the smooth and effective delivery of disability support services, within our core team, focusing on accommodation services. You will be responsible for coordinating and managing service delivery to individuals with disabilities, working closely with a team of dedicated support staff. This role requires a compassionate and organized professional who is committed to making a positive impact on the lives of those we serve.
**Key Responsibilities**:
- Coordinate and schedule support services for individuals with disabilities, ensuring adherence to established care plans and service standards.
- Coordinate accomodation needs for properties that are owned and run by the organisation
- Collaborate with support staff, clients, and their families to assess needs and develop personalised care plans.
- Monitor and evaluate the quality of service delivery, identifying areas for improvement and implementing corrective actions as needed.
- Maintain accurate and up-to-date client records, including progress notes, assessments, and communication logs.
- Act as a liaison between clients, support staff, and management to address concerns, resolve issues, and ensure open communication.
- Stay informed about industry best practices, regulations, and advancements in disability support services.

Have an understanding of legislative, contractual and internal policy, practice and reporting requirements in the NDIS sector

Are self-motivated, well organised, and resourceful. possess a high level of integrity, passion and reliability. Are an advocate for independence.
**Participation as a Team Member**
Contribute to a well-functioning team environment within the organisation
Provide mentoring, support and training assistance for new or less experienced team members
Use initiative when participating at all levels of the organisation and provide creative suggestions as the opportunity arises.
To participate in Reflection and Development and training both internally and externally as required.
**Job Types**: Full-time, Part-time
**Salary**: $65,000.00 - $75,000.00 per year
