**Full Job Description**

Independent Ability Care is currently seeking a highly motivated, energetic, and caring Support coordinator to join our team and work alongside our current Support coordinators.

As a Support Coordinator you will be responsible to promote empowerment, choice and control and community inclusion of people with disabilities and their families, consistent with their NDIS plan.

**About the role**
- Conduct risk assessments with the client, based on their NDIS goals and the level of service defined in the NDIA plan.
- In conjunction with the client and their family, develop a support plan that clarifies the actions to be taken and outcomes to be achieved, where appropriate.
- Coordinate services as defined in the client’s NDIA plan, using a person centred
- based and capacity building approach. This includes informal supports, mainstream services (housing, education, health), specialist services, and NDIS funded supports.
- Monitor the use of support coordination hours and update the Manager if additional hours are required.
- Assist and empower the client and their family by providing information about the range of mainstream and specialist services, including advocacy services available to them to help support clients in achieving their goals.
- Provide Independent Ability Care the Service and Support profile report template in a timely manner and provide relevant progress reports towards achieving plan goals, and recommendations about the appropriate type of support required in the next plan with reasoning.
- Maintain complete case notes and gather record of relevant information about other services accessed by client in continuity of support and review

**Requirement**

Diploma of Community Service

**Job Type**: Part - time (3 days) transitioning to full-time as workload increases

**Salary**: $35.00 - $38.00 per hour

Schedule:
