Dementia Transition Support Specialist

- Collaborate with families, caregivers, and clinical staff to understand how dementia affects each individual.
- Provide training and coaching to carers and clinicians to prevent and respond to behaviour changes through the development of resources and behaviour support planning.
- Develop contextualised reports that drive change and improve quality of life for people living with dementia.
- Conduct assessments to gain a deeper understanding of the person's holistic experience and their care network.
- Work in partnership with the broader organisation to build capability across the home and aged care sectors.
- Contribute to research, projects, and programs, innovating to meet the changing needs of the sector.
Key Requirements:
- Expertise in supporting people living with dementia.
- Experience in Australian residential aged care and/or community aged care.
- Ability to share knowledge and facilitate change at all levels.
- High-level negotiation, communication, and relationship-building skills.
- Excellent problem-solving and decision-making skills.
- Ability to prioritise tasks and meet deadlines.
- Excellent written and verbal communication skills.
- Strong stakeholder management skills.
- A current Australian driver's licence and willingness to travel.
- Empathy and patience.
- Relevant qualification in Allied Health, Nursing, Aged Care, or Mental Health is essential.
