An exciting opportunity exists to provide care and support to individuals living with dementia as they transition from hospital settings back to their homes or into residential care.

Job Responsibilities:
  • Provide on-the-ground support and implement relationship-based support strategies, modelling and coaching with clients, carers, and care staff.
  • Work collaboratively with peers to develop aged care staff and carer skills, expertise, and applied knowledge to understand the person living with dementia and develop best practice support and care.
  • Support the implementation of recommendation plans on the ground, drawing on modelling and coaching techniques, and seek guidance as required.
Requirements:
  • Strong organisational abilities, including effective time management and ability to prioritise in a busy work environment.
  • Ability to work both independently and as part of a team.
  • Demonstrated commitment to customer service principles and conflict resolution.
  • A current driver's licence and safe driving record.
  • Willingness to travel and work intrastate and interstate.
  • Qualification in relevant field; Allied Health assistant, Diversional Therapy, Enrolled Nursing (EN), Aged Care, Cert III or IV in Individual Support / Leisure and Health or similar is highly desirable.
