We are seeking a highly skilled Dementia Support Specialist to join our team.
Job Description:This is a rewarding opportunity to be involved in a dementia centre initiative looking to support people living with dementia who are transitioning from hospital settings back to their homes or into residential care. Our specialist will work closely with families, caregivers and clinical staff to understand how dementia impacts each individual and equip them with the knowledge and skills needed to provide effective support.
Key Responsibilities:- Educate and Support: Provide training and education to carers and clinicians on how to prevent and respond to behaviour changes through modelling, coaching and development of resources including behaviour support planning.
- Assessment and Reporting: Conduct assessments that help to understand the holistic experience of the person and their care network, and develop reports that are contextualised to individual situations and people, effecting change on the ground and improving quality of life for people living with dementia.
- Expertise: Have expertise in supporting people living with dementia.
- Experience: Have experience in Australian residential aged care and/or community aged care.
- Communication Skills: Be relatable and able to share knowledge in a way that supports change at all levels, possessing high level negotiation, communication and relationship building skills.
