Dementia Initiative Opportunity

Support individuals living with dementia as they transition from hospital settings to their homes or residential care.

  • Apply assessment and biopsychosocial approaches to prevent, minimise or respond to changes in behaviour related to dementia.
Key Responsibilities:
  • Collaborate with families, caregivers and clinical staff to understand the impact of dementia on each individual.
  • Equip carers and clinicians with skills to prevent and respond to behaviour changes through coaching, development of resources and behaviour support planning.
  • Provide recommendations that are relationship centred and informed by evidence-based best practice, synthesising information from observation and written documentation.
  • Develop reports that effect change on the ground and improve quality of life for people living with dementia, contextualised to individual situations and people.
  • Conduct a range of assessments to understand the holistic experience of the person and their care network.
  • Partner with stakeholders to build capability across the home and aged care sectors.
  • Contribute to research, projects and programs to meet the changing needs of the sector.
  • Undertake other assignments as required and aligned with areas of skill, expertise and knowledge.
