Chronic Disease Management Nurse

Clinicians with a passion for delivering high-quality patient care and improving health outcomes are encouraged to apply for this part-time Chronic Disease Management Nurse role at our Narre Warren Medical Centre.

The successful candidate will be responsible for working closely with General Practitioners and patients to identify and manage chronic conditions, promoting preventative health and reducing hospital admissions.

Key Responsibilities:
  • Develop comprehensive care plans and team care arrangements to support patients in managing their diseases.
  • Empower patients to take ownership of their health through coaching and education.
  • Conduct health assessments to identify chronic disease early and provide recommendations for ongoing care.
  • Collaborate with healthcare professionals to promote preventative healthcare throughout the practice.
Required Skills and Qualifications:
  • AHPRA registration as an Enrolled or Registered Nurse.
  • Proven experience in Chronic Disease Management and health assessments.
  • Knowledge of Health Assessment is highly regarded.
  • Strong organisational and time management skills.
  • Exceptional communication and customer service skills.
