Chronic Disease Management Nurse

Key Responsibilities:

  • Conduct thorough health assessments to inform treatment decisions and promote preventative healthcare.
  • Provide education and support to patients on managing chronic conditions and adhering to treatment plans.
  • Work closely with multidisciplinary teams to ensure seamless care coordination and optimize patient outcomes.
Requirements:
  • AHPRA Registered Nurse with experience in Chronic Disease Management and health assessments.
  • Proven ability to work effectively in a fast-paced environment and prioritize tasks efficiently.
  • Strong communication and interpersonal skills to build rapport with patients, families, and healthcare professionals.
  • Ability to work autonomously and as part of a team to achieve common goals.
  • Knowledge of health assessment and chronic disease management principles highly regarded.
