
Palliative Care Coordinator

This is an exciting opportunity to join our innovative care service as a Virtual Care Specialist, where you will play a pivotal role in coordinating and delivering palliative care to patients and their families.
Job Description:
The successful candidate will provide comprehensive care planning and coordination for patients and carers, liaising with community and in-patient palliative services. They will offer expert clinical advice to patients, carers, and healthcare professionals, develop and implement effective care plans, and organise referrals while collating clinical information. Additionally, this role involves developing clinical pathways to optimise service delivery and innovate best practice models of care tailored to the patient journey.
Required Skills and Qualifications:
- Current registration with the Nursing and Midwifery Board of Australia as a Registered Nurse (Division 1)
- Minimum 5 years full-time post-graduate experience with at least 3 years full-time equivalent experience in Palliative Care Nursing
- Approved post-graduate qualification or evidence of working towards Masters level qualification relevant to Palliative Care Nursing
- Clinical leadership skills and ability to work collaboratively within a multidisciplinary framework
- Effective communication skills and ability to contribute nursing expertise to improve care within a multidisciplinary team
