The role of the Aboriginal Palliative Care Coordinator (APCC) is to support the SLHD Palliative Care Service in providing respectful and culturally appropriate end-of-life and palliative care for Aboriginal people. The APCC will work collaboratively with clinicians, community-based service providers, and patient families at the primary health care level. This position acts as a conduit for Aboriginal patients with palliative care needs in both hospital and community settings, ensuring culturally competent and culturally safe practices are integrated into care coordination. This role will be working across SLHD.

Ideal Candidate- This is an Identified Aboriginal / Torres Strait Islander Position. Applicants must be of Aboriginal descent through parentage, identification as Aboriginal, and accepted in the community as such. Exemption is claimed under Section 14 of the Anti-Discrimination Act 1977.
- Understanding of issues affecting Aboriginal people related to grief, loss, social, and emotional wellbeing.
- Ability to lead the development of culturally safe environments for Aboriginal patients and their families within a multidisciplinary team.
- Holds a current NSW Driver's Licence (P1 and P2 acceptable).
