Job Title

Clinical Nurse Specialist Role


A Clinical Nurse Specialist will be responsible for coordinating, leading and participating in projects related to patient flow and care coordination across services. The successful candidate will work within a dynamic team to provide quality care to community clients of the Palliative Care Service. They will also provide evidence-based nursing care to diverse populations, including individuals, families and communities.

Key Responsibilities:
  • Coordinate, lead and participate in projects related to patient flow and care coordination across services.
  • Monitor patient wait times and discharge dates via electronic patient journey boards, identify key 'Waiting for What' (W4 W) and patient exit blockers.
  • Lead multidisciplinary meetings to ensure timely patient care, advise referrers and escalate bed blocks.
  • Work within a dynamic team to provide quality care to community clients of the Palliative Care Service.
  • Provide evidence-based nursing care to diverse populations, including individuals, families and communities.

About You

We are seeking an experienced Registered Nurse with a strong background in palliative care. The ideal candidate will have at least three years' experience in palliative care and relevant post-registration qualifications. They will also possess excellent communication and interpersonal skills, as well as the ability to work within an interdisciplinary team.

Requirements

To be considered for this role, you must:

  • Be currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency
  • Have a current NSW driver's license (Class C or above)
  • Have at least three years' experience in palliative care
  • Hold appropriate Australian work visas (if applicable)
