Palliative Care Coordinator Role

We are seeking a highly skilled Palliative Care Coordinator to join our multidisciplinary team at St Vincent's Hospital, Darlinghurst.

This is an exciting opportunity for a registered nurse with experience in palliative care to take on a leadership role in the coordination of nursing and healthcare.

You will be working closely with our team to provide evidence-based nursing care to people of all ages and cultural groups.

The successful candidate will have excellent communication and interpersonal skills, with the ability to work effectively in a dynamic environment.

Key responsibilities include:

  • Coordinate, lead and participate in projects related to patient flow and care coordination from one service location to another.
  • Monitor patient wait times and estimated discharge dates through update and management of electronic patient journey boards as well as identifying key 'Waiting for What' (W4W) and patient exit blockers.
  • Lead multidisciplinary meetings in relation to patient flow and movement, to ensure that patients are seen in a timely manner. Advise and negotiate with referrers, and escalate any bed blocks.
  • Work in a dynamic team leading our approach to the provision of quality care to community clients of the Palliative Care Service.
  • Provide evidence-based nursing care to people of all ages and cultural groups, including individuals, families and communities.
  • Take an extended leadership role in the coordination of nursing and health care, providing expert nursing assessment and triage to facilitate optimal health outcomes. Feed back to staff on how to improve patient satisfaction or efficiency.

About You:

  • Currently registered as a Registered Nurse (Division 1) with the relevant Australian Health Professionals Regulation Agency.
  • Currrent NSW driver's licence (Class C or above).
  • Relevant post-registration qualifications and at least three years' experience working in palliative care.
  • Demonstrated ability to work within the interdisciplinary team at a senior level with a commitment to ongoing professional development and demonstrated effective communication and interpersonal skills.
  • Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis in their daily practice as a registered nurse with effective time management skills and ability to prioritise competing workloads.
