Clinical Nurse Specialist

The Clinical Nurse Specialist is a pivotal role within our Palliative Care Service. This position plays a vital part in enhancing patient care and outcomes through effective coordination and leadership.

Key responsibilities include:

  • Developing and implementing projects to optimize patient flow and care coordination across service locations.
  • Monitoring patient wait times and estimated discharge dates, identifying key areas for improvement.
  • Leading multidisciplinary meetings to ensure timely patient assessments and smooth bed allocation.
  • Working closely with the team to provide high-quality care to community clients of the Palliative Care Service.
  • Providing evidence-based nursing care to individuals, families, and communities.
  • Coordinating nursing and health services, delivering expert assessment and triage to achieve optimal health results.
Essential Skills and Qualifications
  • Proven expertise in clinical nursing, including patient assessment, diagnosis, and treatment.
  • Strong leadership and coordination skills, with ability to work effectively in a dynamic team environment.
  • Excellent communication and interpersonal skills, with capacity to negotiate and collaborate with referrers and staff members.
  • Ability to analyze complex information, identify trends, and develop strategies to improve patient outcomes.
  • Sound knowledge of palliative care principles, practices, and legislation.
