The Care Coordinator role at Ingenia Communities plays a vital part in delivering a service that connects residents with support to live an active, independent life in our communities. The primary responsibility is to identify residents' needs, obtain approval for government-funded care and support services, and arrange for these services to be delivered to residents in their homes.

As a Care Coordinator, you will deliver a service focused on assisting elderly residents to remain as independent as possible by connecting them with care and support services. These services include personal care, domestic assistance, nutrition, mobility and transportation, clinical care, after-hospital care, respite care, vision and hearing assessments, medication management and reviews, and palliative care.

Main duties include registering care applications, managing client intake processes, determining client needs, arranging assessments and funding approvals, connecting clients with care providers, record management, and traveling to villages for face-to-face assessments and building relationships with residents.

This role would suit an experienced Aged Care Assessment professional looking to transition from the hospital system to an over-the-phone support role. The ideal applicant will have current AHPRA registration, practical experience working in the health and/or aged care industries, excellent care assessment and planning experience, strong leadership and people management skills, a positive and compassionate nature, great customer service skills, and a strong attention to detail.
