Care Coordination Specialist Role

We are seeking an experienced Care Coordination Specialist to join our Radiation Oncology team at GenesisCare. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality patient care and ensuring seamless coordination of clinical services.

Key Responsibilities:
  • Provide excellent patient care, prioritizing their physical and psychological needs.
  • Undertake direct patient care activities, including education, wound management, and medication administration, as per established protocols.
  • Effectively manage nursing patient load, balancing workload and ensuring timely completion of tasks.
  • Facilitate effective communication with doctors, patients, physics, and radiation therapy staff, providing feedback as relevant.
  • Liaise with co-located St. Vincent's Campus - Private and Public Hospital nursing staff to ensure coordinated care.
  • Document, communicate, and action patient listed events promptly and accurately.
  • Maintain procedures, protocols, practices, and clinical guidelines aligned with best practice radiation oncology nursing standards.
  • Adhere to medication management protocols and establish clear communication channels for allied health services using ISBAR.
  • Ensure reliable, motivated, and flexible performance in meeting team objectives.

Requirements:
