As a Senior Care Coordinator, you'll play a vital role in delivering person-centered care coordination to our clients. Your responsibilities will include attending consultation visits, conducting intake assessments, developing care plans, and managing and monitoring client wellbeing.

You'll collaborate closely with our service coordination team to ensure the comprehensive support your clients require.

  • Experience in service delivery and coordination within a care setting.
  • Strong leadership skills with the ability to provide effective feedback.
  • Excellent communication and interpersonal skills.
  • Proactive and empathetic approach to service delivery.
  • Problem-solving and conflict resolution skills.
  • Understanding of WH&S risks and relevant legislative requirements.
  • Qualification in Nursing or Allied Health highly regarded

$55,000 - $65,000 per annum, depending on experience.
