Care Coordinator

Key responsibilities include providing holistic and person-centered care to residents, ensuring their individual needs, wishes, and goals are met within a supportive environment.
About the RoleThe role of Team Leader plays an integral part in delivering high-quality care to residents at all times. They work closely with Clinical Care Coordinators and Registered Nurses to oversee and coordinate care across the home while actively ensuring that each resident feels valued, respected, and treated with dignity.
Key Responsibilities- Support the coordination of care needs for residents, including medication management, nutrition, and hydration, as well as pressure care management.
- Lead/supervise care on the shift, focusing on designated wings or areas.
We seek individuals who possess:
- A relevant qualification: Diploma of Enrolled Nursing (with APRHA Registration to administer medication) or Certificate IV in Ageing Support (including HLTHPS007 Administer & Monitor Medicines).
- A commitment to excellence, integrity, and trustworthiness.
- An ability to effectively communicate at all levels, both verbally and in writing.
- The capacity to understand and utilize information technology and programs, as well as learn new skills as required.
