Care Coordinator Position

Summary of Role:

We are seeking a skilled Care Coordinator to work in our Home Care Packages team. The successful candidate will be responsible for visiting clients at their homes, conducting comprehensive assessments, and developing care plans. They will also be required to set up supports and services, review care arrangements, and manage connections with relevant parties.

Main Responsibilities:

  • Conducting home visits to assess client needs and develop care plans
  • Identifying client goals and requirements, creating schedules for services, and arranging necessary supports
  • Coordinating the setup of services, including explaining service agreements and bookings to clients
  • Regularly reviewing care arrangements and making adjustments as needed
  • Managing connections with support services, verifying charges, and communicating with service providers regarding any discrepancies
  • Matching care staff with clients based on their needs and circumstances
  • Serving as the primary point of contact for clients, families, healthcare professionals, and other external providers

Key Requirements:

  • Excellent verbal and written communication skills
  • Effective time management skills to handle workload and meet deadlines
  • Strong interpersonal skills, including building and maintaining relationships within the team and with clients
  • A valid Australian Driver's Licence and access to a vehicle
  • Strong planning abilities and capacity to manage competing priorities
  • Attention to detail and organizational skills
  • A commitment to delivering high-quality support and services to clients
