Care Coordinator

As a dynamic and experienced Care Coordinator, you will work within a client-directed framework to ensure the wellbeing of older people who receive Home Care Packages.

Your responsibilities will include:

  • Developing, reviewing, and monitoring care plans that meet individual needs and goals.
  • Collaborating with internal and external services partners to deliver a holistic service.

To be successful in this role, you will need:

  • An understanding of the social, psychological, and physical aspects of ageing and disability.
  • Experience in community care, particularly care/case management.
  • Knowledge and skills in behaviour management strategies.
  • The ability to be flexible, innovative, and independent.

You must have:

  • A relevant tertiary qualification and/or extensive experience in human services.
  • A current driver's license.
  • A current NDIS worker screening check or willingness to obtain.
  • A current NDIS worker orientation module certificate or willingness to obtain.
  • Full vaccination for COVID-19 and meet the requirements of the COVID-19 Vaccination Policy.
