Job Summary:

Pearl Home Care is seeking a skilled professional to fill the role of Case Manager. As a key member of our team, you will be responsible for coordinating and managing consumer care across the Ballarat and Geelong & Bellarine Peninsula regions.

Key Responsibilities:

  • Case Management
    - Conduct thorough assessments to identify unique consumer needs, preferences, and goals.
    - Develop and implement individualized care plans that address these needs, updating as necessary.
    - Foster effective communication among multidisciplinary teams, including healthcare professionals, to deliver high-quality care.
    - Regularly monitor and evaluate consumer progress, adjusting care plans as needed, and reporting changes or concerns to the healthcare team.
    - Ensure all care services adhere to relevant regulations, policies, and standards of care, participating in quality improvement initiatives.
  • Safety and Risk Management
    - Implement safety protocols and procedures to mitigate risks in the home care environment.
    - Provide regular safety training to staff, ensuring they understand safety policies and can work safely.
    - Investigate incidents and accidents, taking action to prevent similar incidents from occurring.
  • Business Development and Partnerships
    - Establish and maintain relationships with key stakeholders, including healthcare professionals and community organizations.
  • Operational Excellence
    - Oversee and coordinate nursing care activities, such as wound care and clinical procedures.
    - Assess and monitor consumer vital signs, adjusting the care plan as needed.
    - Collaborate with the healthcare team to ensure holistic and coordinated care.

Requirements:

  • Qualifications:
    - Relevant degree in a field such as social work, nursing, or healthcare management.
    - Proven experience in case management, healthcare, or a related field.

    - Strong problem-solving and analytical skills.
