Job Title: Care Coordinator
">We are seeking a skilled professional to assist clients in remaining safely at home by assessing their needs and developing individualized plans.

- Utilize expertise in the CHSP program to assess clients and create personalized care plans that promote independence and safety.
- Collaborate with a multidisciplinary team to deliver high-quality care and support to clients.
- Nurture relationships with clients, carers and other stakeholders to ensure effective communication and coordination of care.
- Conduct home visits and assessments to identify client needs and develop care plans.
- Provide education and support on topics such as ageing, disability, health and wellbeing.
- Participate in regular meetings to discuss client progress, share best practices and address challenges.
Requirements:
To succeed in this role you will require excellent time management, organisational and communication skills. You must also be compassionate, flexible and have a positive approach to client care.
You will need to nurture friendly, respectful relationships and contribute to a collaborative team environment.
A current Police Record Check and NDIS workers Screening Check is required.
