Job Overview

We are seeking a dedicated and compassionate Social Worker to join our team in the Connected Care program. As a key member of our interprofessional team, you will provide vital support to clients enrolled in the service, delivering consumer-centred care through home visits, telehealth, phone, or in-clinic appointments.

About the Role

This role includes direct client care, secondary consultations to support team members, and contributing to the development of an agile model of care that delivers excellent health outcomes. You will also play a key role in care coordination as part of a multidisciplinary team.

Key Responsibilities

Requirements

To be successful in this role, you will require:

  • Ability to work collaboratively within a team environment.
