Community Care Specialist Role

This position involves delivering comprehensive, flexible, and culturally sensitive health services to Aboriginal families with children aged 0-8 years accessing healthcare facilities.

The New Directions program is a multidisciplinary team comprising Aboriginal Health Workers/Practitioners, Social Workers, and Dietitians. A key focus for the program is to foster positive connections between the community and healthcare services, as well as partner with the community to achieve Closing the Gap in health outcomes for Aboriginal & Torres Strait Islander community members.
