Job Title: Aboriginal Care Specialist

This is a pivotal role that supports patients and their families throughout the cancer care journey. It's an opportunity for Aboriginal or Torres Strait Islander people to join our team in making a difference.

About the Role:
  • Support patients and families to navigate cancer services with dignity and respect.
  • Provide leadership in care coordination across clinical units, services, and community providers.
  • Advocate for improved patient experiences by embedding culturally safe practices.
Candidate Requirements:
  • A cultural connection to Tamworth and the surrounding Aboriginal community.
  • Knowledge of Aboriginal health services and the barriers faced when accessing care.
  • A passion for improving Aboriginal health outcomes and patient experiences.
  • Strong communication, relationship-building, and advocacy skills.
  • Relevant qualifications or experience in health, social science, or community service.
Key Responsibilities:
  1. Support patients and families to navigate cancer services with dignity and respect.
  2. Provide leadership in care coordination across clinical units, services, and community providers.
  3. Advocate for improved patient experiences by embedding culturally safe practices.
  4. Build partnerships with community organisations, Aboriginal healthcare workforce, clinicians, and service leaders.
  5. Facilitate communication between patients, families, and healthcare teams to improve health outcomes.
  6. Support transition, planning, and evaluation of programs that enhance service delivery for Aboriginal people.
