
Palliative Care Nurse Specialist

Clinical Nurse Specialist - a pivotal role in palliative and supportive care. This position plays a key part in delivering compassionate, coordinated care within the Supportive and Palliative Care service. The Clinical Nurse Specialist guides patients and carers through the care continuum with dignity and support from hospital to home.
Key Responsibilities- Deliver advanced clinical nursing care with autonomy and in collaboration with multidisciplinary teams.
- Provide care coordination to ensure a seamless journey through the care continuum from hospital to home.
- Lead a multidisciplinary team to improve patient outcomes by championing, educating and demonstrating best practice.
- Demonstrate problem solving and decision-making according to best practice standards.
- Communicate effectively with patients, family/carers and a diverse range of healthcare providers.
- Current registration with a relevant nursing authority – Division 1 Registered Nurse.
- Post-registration qualifications in palliative care are highly valued.
- A minimum of 3 years full-time equivalent experience in palliative care nursing is required.
- A current driver's license is necessary for work-related travel.
