Social Worker

We are seeking a skilled Social Worker to join our team at Central Australian Aboriginal Congress in the Social and Emotional Wellbeing (SEWB) Service.
The SEWB team provides comprehensive care, including medical, therapeutic, and cultural support, with a focus on intensive case management.
The successful candidate will assist individuals and their families to access evidence-based social work methodologies, including screening and assessments, case management, advocacy, outreach, and supportive counseling.
The role requires a strong commitment to cultural sensitivity and an understanding of the complex needs of our clients, who may experience co-existing challenges such as general and mental health issues, family and community violence, and substance abuse.
The Social Worker will work closely with the SEWB Manager and other team members to develop and implement individualized care plans, ensuring seamless access to community services and social opportunities.
Key responsibilities include:
- Providing psychosocial supports to restore socio-economic and cultural functioning to clients referred to the SEWB service.
- Assisting individuals and families to access community services, including employment and social opportunities.
- Developing and implementing individualized care plans, in collaboration with the SEWB Manager and other team members.
- Providing case management, advocacy, outreach, and supportive counseling services.
