Job Summary

We are seeking a skilled Social Worker to join our Continuing Care Team in Illawarra Community Mental Health. As a key member of our multidisciplinary team, you will provide high-quality evidence-based clinical services to mental health consumers, including assessment, care coordination, intervention, and review.

Key Responsibilities
  • Provide comprehensive assessments and develop individualized care plans in collaboration with consumers, families, and healthcare professionals.
  • Coordinate and deliver evidence-based interventions, including therapy and support services, to promote recovery and well-being.
  • Work effectively with consumers, families, and services to provide strengths-based assessment, treatment, and management.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and service delivery.
  • Develop and maintain strong relationships with consumers, families, and community services to promote a recovery-focused approach.
Requirements
  • Approved tertiary qualifications in Social Work and current membership of the AASW.
  • Demonstrated experience in mental health assessment, risk assessment, planning, and intervention.
  • Commitment to a consumer-focused recovery-oriented approach and collaboration in care planning.
  • High-level communication skills, both written and oral.
  • Computer skills to support the position and NSW Health requirements.
  • Time management skills and ability to prioritize tasks effectively.
  • Current unrestricted Class C driver's license.
