Job Summary
The Illawarra Shoalhaven Local Health District is seeking an Aboriginal Health Practitioner/Aboriginal Health Worker to join our team. This is a permanent full-time position that offers a competitive salary and a range of benefits.
About the Role
The successful candidate will work collaboratively with healthcare service providers across the district to increase access to health treatment and prevention services for Aboriginal people. They will also address issues related to the management of chronic disease, with a focus on improving patient outcomes and experiences within Aboriginal communities.
Key Responsibilities
- Initiate, undertake, and interpret findings from a range of assessments, tests, and procedures.
- Work with medicines in line with legislative, regulatory, and organisational requirements.
- Work within a multidisciplinary health team, under direct or indirect supervision, to provide routine primary healthcare practices and procedures.
- Manage the care of Aboriginal people with or at risk of developing chronic disease.
Requirements
- Must be of Aboriginal and/or Torres Strait Islander descent and have demonstrated knowledge of current Commonwealth, State, and Local Aboriginal health priorities.
- Hold a Certificate IV in Aboriginal Primary Health Care (Practice) and a current authority to practice as an Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA).
- Demonstrated ability to deliver, evaluate, and report on health promotion and education programs.
- Demonstrated ability to engage and effectively prioritise workloads under limited supervision within a multidisciplinary team.
- Demonstrated skills in client assessment, support, referral, and advocacy in health or related fields.
- Well-developed verbal communication skills and ability to work effectively with a range of health professionals and service providers.
- Demonstrated computer skills and ability to maintain accurate records in clinical and non-clinical information systems.
- Current driver's licence Class C and ability and willingness to travel within the LHD for work purposes.
