About the RoleWe are seeking an experienced and compassionate Community Health Advocate to join our team of dedicated professionals. The successful candidate will be responsible for:
- Providing outreach services to clients, including planned transports and cultural input to service delivery;
- Servicing as a liaison between clients and healthcare providers, ensuring seamless communication and coordination;
- Supporting clients with healthcare follow-ups, advocating on their behalf when necessary;
- Collaborating with other healthcare professionals to develop and implement client-centered care plans;
- Conducting short-term case management and referrals to relevant services as required;
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders, including clients, families, and healthcare providers. They will also have a strong understanding of the social determinants of health and the importance of cultural competency in healthcare delivery.

Key Requirements
