Aboriginal Community Health Worker

We are seeking an Aboriginal Community Wellbeing Worker to join our team at Central Australian Aboriginal Congress.
The successful candidate will provide a range of services including outreach to clients, planned transports, service brokerage, cultural input to service delivery, client support for healthcare follow-ups, healthcare advocacy, client follow-ups for missed appointments, and short-term case management.
This role is critical for the promotion of initial and continued access to holistic primary healthcare.
As part of this role, you will be supported to attain qualifications in an area such as alcohol and other drugs, community services, or mental health.
Key responsibilities will include:
- Providing outreach services to clients in remote communities
- Coordinating planned transports for clients
- Providing cultural input to service delivery
- Supporting clients with healthcare follow-ups
- Advocating for clients' healthcare needs
- Following up with clients for missed appointments
- Providing short-term case management
We are looking for a candidate who is passionate about working with Aboriginal communities and has a strong understanding of the social and cultural determinants of health.
