We are seeking an Aboriginal Community Wellbeing Worker to join our Social and Emotional Wellbeing (SEWB) team, working in a rewarding role that makes a tangible difference in people's lives.

The ideal candidate will possess strong interpersonal and communication skills, allowing them to build trust and rapport with clients from diverse backgrounds. Proficiency in cultural competency and understanding of social determinants of health is essential for providing tailored support and connecting individuals with relevant resources.

This role offers a range of exciting challenges, including:

  • Providing outreach services to clients, conducting planned transports, and facilitating access to healthcare services;
  • Offering service brokerage and advocacy to ensure seamless transitions between care providers;
  • Culturally sensitive input to service delivery, enhancing the overall client experience;
  • Supporting clients with healthcare follow-ups, empowering them to manage their health effectively;
  • Facilitating short-term case management, focusing on addressing immediate needs and preventing potential crises.

  • A base salary of $66,963 - $72,559 per annum;
