This role is focused on bridging clinical expertise and compassionate care to enhance the lives of people living with dementia.
Job RoleThe Clinical Lead in Dementia Care plays a vital part in a small team that provides support to people living with dementia. The team collaborates closely with other teams, including those providing home support assessments, ACAT assessments, community dementia nursing, neuropsychology services, and carer support.
The successful candidate will have excellent communication skills, be able to prioritize workload, and demonstrate professionalism in an empathetic and caring manner. They will also have knowledge of aged care and referral pathways, as well as understanding of dementia and person-centred care.
This position involves promoting local community awareness of dementia, providing information to people with dementia and their carers, leading local dementia carer support groups, and providing dementia education to various stakeholders. The successful candidate will work collaboratively with clients, carers, and service providers to achieve identified outcomes.
Main Responsibilities:- Promote local community awareness of dementia and its impact
- Provide relevant information to people with dementia and their carers
- Lead local dementia carer support groups and facilitate peer-to-peer support
- Develop and deliver dementia education to various stakeholders
The successful candidate will work in collaboration with key stakeholders to ensure seamless delivery of services. This is a temporary part-time position until June 2027, working 24 hours per week.
