Dementia Care Specialist
We are seeking a passionate and experienced Dementia Care Specialist to provide information and support to clients living with dementia and their families/carers.
About the Role:
- Provide expert advice on dementia, available services and resources to people living with dementia, their families and carers
- Develop individualised care plans that reflect the client's needs, goals, preferences, and promote independence using the Well-Living approach
- Equip clients, families and carers with the knowledge they require to exercise autonomy and make informed decisions about their care and services
Key Responsibilities:
- Assess and identify the client's needs, strengths and preferences
- Collaborate with other professionals to develop and implement comprehensive care plans
- Deliver educational sessions and workshops on dementia awareness and management

Qualifications and Skills:
- Relevant tertiary qualifications in Aged Care/Dementia Practice or equivalent qualification
- Experience providing support to, and advice about dementia to people living with dementia, their family and carers
- Comprehensive knowledge about dementia, including key features and potential implications trajectory
