
Care Coordinator for Vulnerable Populations

The Community Transition Clinician plays a pivotal role in facilitating successful transitions of individuals with severe mental illness from custody to the community.
This position requires strong clinical skills, discipline-specific knowledge, and the ability to provide collaborative care that addresses patients' complex needs.
">Key Responsibilities- Link patients to relevant community services and support their reintegration through general care coordination and specialized skills.
- Assess patients' clinical, psychosocial, and behavioral vulnerabilities to deliver tailored care.

">Requirements- A current unrestricted Authority to Practice as a registered nurse or registration with the relevant professional board (Occupational Therapy Board of Australia/ Psychology Board of Australia) and at least 3 years' experience working in mental health settings.
Candidates should possess excellent communication and interpersonal skills, as well as the ability to work collaboratively within a multidisciplinary team.
