
Vulnerable Care Specialist

We are seeking an experienced professional to partner with vulnerable individuals who are at risk of homelessness, have experienced homelessness, or are impacted by hoarding and/or squalor.

- Collaborate in a supportive team environment
As part of the Vulnerable Persons team, you will use your extensive skills and experience to oversee the case management of aged care clients in their homes, ensuring they have a flexible, responsive support service/intervention tailored to their individual needs and interests.
Key Responsibilities:
- Support those who are homeless or at risk of homelessness
- Facilitate collaborative service approaches to identify supportive solutions for people experiencing hoarding, squalor, or clutter
- Assess, coordinate, and review client care needs within allocated funding
- Coordinate services to ensure housing is maintained for older persons and their care needs are met so they can continue living in the community
- Deliver creative strategies and solutions that deliver exceptional client outcomes
- You will be responsible for ensuring the delivery of flexible and responsive services tailored to an individual's needs and interests, while also providing advocacy and referral services to clients
- Collaborate with government and non-government agencies within the community care sector to achieve improved housing, health, and wellbeing outcomes for clients
- Demonstrated experience in case managing individual clients is essential
- High level of communication skills and a 'no judgment' approach
- Understanding of the complexities associated with situations of hoarding and/or squalor highly desirable
- Homelessness and mental health experience highly desirable
- Relevant qualifications in allied health, nursing, social science, or related fields is desirable
