Care Coordinator Specialist

About the Role:

This is a specialist position that coordinates care for individuals with complex mental health needs.

We support people to access services across seven key domains, including Mental Health, Aboriginal Health, and Homelessness.

Key Responsibilities:
  • Develop individualized care plans in collaboration with patients and their support networks.
  • Coordinate services to improve health outcomes and reduce hospitalization.
Requirements:
  • Experience working in a fast-paced environment.
  • Demonstrated ability to deliver high-quality care coordination services.
  • Knowledge of adult mental health principles and practices.
Qualifications:
  • Mental Health Nurse, Social Worker, Occupational Therapist, or Psychologist registration.
  • Current registration with the relevant authority.
  • Working with Children's Check (WWCC).
