About the Role

We are seeking a highly skilled and compassionate Case Manager to join our team at Anglicare North Queensland Limited. As a Specialist adult Homelessness and Crisis Accommodation Services provider, we offer Temporary Supported Accommodation and support to Men and Women 18 years and above who are homeless or at risk of homelessness.

Key Responsibilities
  • Develop and implement holistic case management plans to support clients in achieving stability in their living arrangements.
  • Build and maintain relationships with stakeholders, including government bodies, community organisations, and other service providers to provide efficient and effective services to clients.
  • Conduct regular assessments and reviews to ensure clients are receiving the necessary support and services.
  • Collaborate with other divisions to create exceptional client experiences and achieve successful outcomes.
  • Ensure compliance with organisational programs, policies, and operations, as well as relevant government regulations and funding requirements.
Requirements
  • Proven case management experience using a holistic and client-centered approach to managing a caseload.
  • Extensive experience working in government-funded programs with strict budgetary and contractual requirements of service.
  • Knowledge of organisational programs, policies, and operations.
  • Industry-relevant qualifications or relevant industry experience.
  • Experience working with Aboriginal and Torres Strait Islander people, families, and communities will be highly regarded.
  • Experience working with people from culturally and linguistically diverse (CALD) backgrounds will be highly regarded.
