We are seeking an experienced and skilled Care Manager to join our team at The myHomecare Group. As a Care Manager, you will play a critical role in providing high-quality care to our clients in Cairns.
Key Responsibilities- Care Provision and Management: Maintain a high standard of care provided by our providers and staff, ensuring our clients receive the best possible support.
- Leadership and Collaboration: Lead from the front in this management position, working closely with our Nurses, Regional and Portfolio Managers to develop and shape our service offerings.
- Care Planning and Coordination: Develop, monitor, and reassess care plans in consultation with clients, their families, and other healthcare professionals.
- Team Management: Manage and develop a team, including the Assistant Care Manager and support staff, to ensure seamless service delivery.
- Client Service Delivery: Manage client service delivery across all levels under the government-funded CHSP and homecare packages programs.
- Compliance and Quality: Ensure compliance with all relevant legislative and industry standards, maintaining the highest level of quality in our services.
- Experience: Proven recent experience in case management in Aged Care or Community Services sector.
- Clinical Experience: Clinical experience preferred, with knowledge of Home Care Packages, Commonwealth Home Support Program, and Aged Care Quality Standards.
- Frontline Management Skills: Demonstrated frontline management skills, with the ability to lead and work collaboratively with our schedulers, clinical team, and support staff.
- Certifications: Current First Aid Certificate and current CPR certificate, with the right to work in Australia and a current driver's license, registration, and access to your own car with comprehensive car insurance.
