Dementia Care Adviser Role

We are seeking a compassionate and experienced professional to provide person-centred case management that supports dignity, choice, and wellbeing for individuals living with dementia.

Key Responsibilities:
  • Deliver evidence-based advice and advocacy during home visits and group settings
  • Conduct educational talks to increase understanding of dementia in the community
  • Complete timely assessments and care plans to ensure services are optimised and effective
Requirements:
  • Diploma in Community Services, Dementia Care, or equivalent qualification
  • Strong understanding of dementia, its progression, and its impacts on individuals and carers
  • Experience working with older people, particularly those living with dementia
