Homeless Services Coordinator

We're looking for a dedicated professional who can provide consistent, trauma-informed support and empower individuals on their journey toward long-term, dignified housing.
This is an opportunity to make a real difference in the lives of men experiencing homelessness by working collaboratively with a team to develop and implement strengths-based, solution-focused case plans.
You will also be responsible for supporting transitions to long-term housing through warm referrals and coordinated outreach, advocating across housing, health, legal, and community systems to meet client needs, and collaborating with internal teams and external partners to connect clients with relevant services.
A degree qualification in Social Work, Psychology, or a related field, demonstrated experience in case management within a homelessness or community services setting, knowledge of homelessness, trauma, and housing systems, strong communication and advocacy skills, and cultural awareness are essential requirements for this role.
