Homelessness Support Specialist

We are seeking a dedicated professional to join our team as a Case Manager. As a Case Manager, you will provide tailored support for individuals navigating homelessness.
Your primary responsibility will be to empower clients to set and achieve goals, access long-term housing, and rebuild their lives. You will work collaboratively with internal teams and external agencies to ensure coordinated care.
Key responsibilities include:
- Building strong relationships and developing personalized case plans
- Delivering outreach-based support and facilitating warm transitions into long-term housing
- Advocating on behalf of clients to access health, housing, legal, education, and well-being services
To be successful in this role, you will need:
- A degree in Social Work, Psychology, or a related field
- Experience in case management and supporting people experiencing homelessness
- Knowledge of trauma-informed, strengths-based, and person-centred approaches
