We are seeking a dedicated Case Manager to provide tailored support for men navigating homelessness. As a Case Manager, you will empower individuals to set and achieve goals, access long-term housing, and restore dignity.

- Flagstaff supports up to 64 single men.
- The Open Door accommodates 37 single men.
You will be responsible for providing emotional support, practical assistance, and advocacy to help clients overcome obstacles and achieve stability.
This is a critical role in our mission to empower homeless individuals and provide them with the tools and resources needed to rebuild their lives.
Key Responsibilities:
- Develop and implement individualized plans to address client needs.
- Collaborate with internal teams and external agencies to ensure coordinated care.
- Provide emotional support, practical assistance, and advocacy to clients.
- Monitor client progress and adjust plans as needed.
Requirements:
- Degree in Social Work, Psychology, or related field.
- Experience in case management and supporting people experiencing homelessness.
