We are seeking a highly skilled and compassionate Case Manager to join our team. As a Transitional Housing Case Manager, you will be working with individuals who are experiencing homelessness and facing multiple complex challenges.
Your role will involve providing individualised case management and outreach services to empower men on their journey towards long-term, dignified housing. You will work closely with residents to develop strengths-based, solution-focused case plans, and support transitions to long-term housing through warm referrals and coordinated outreach.
You will also have the opportunity to advocate across housing, health, legal, and community systems to meet client needs, and collaborate with internal teams and external partners to connect clients with relevant services.
This is a permanent full-time position, based in Melbourne, VIC. If you are a driven and empathetic professional who is passionate about making a difference in the lives of others, we encourage you to apply.
Key Responsibilities- Develop and implement strengths-based, solution-focused case plans in partnership with clients
- Support transitions to long-term housing through warm referrals and coordinated outreach
- Advocate across housing, health, legal, and community systems to meet client needs
- Collaborate with internal teams and external partners to connect clients with relevant services
- A degree qualification in Social Work, Psychology, or a related field
- Demonstrated experience in case management within a homelessness or community services setting
- Knowledge of homelessness, trauma, and housing systems (including the Victorian Housing Register)
- Strong communication and advocacy skills
- A collaborative spirit and ability to build positive relationships with clients and stakeholders
